Specific services include:

* Providing health screening and physical assessment
Public Health Nursing
* Nutrition assessment and referrals
* New baby care and bonding
* Referrals for medical care and other resources
* Service coordination
* Health counseling
* Assessments and plans of care
* Assessing the family as a whole for needs, strengths, and resources
* Fall assessments and safety in the home
* Exercise and wellness program referrals
